Lead (Volunteer) Application

All applicants must be CPR & First Aid Certified, Fingerprinted, background checked & virtual Trauma training. No exceptions! All youth (at least age 16), must have their legal guardian/parent complete this application for them.

Liability Release, Medical & Indemnity Agreement: I, (Volunteer/ or as a parent/guardian on behalf of volunteer) hereby understand, agree & consent to serve as a “Volunteer” for Steadfast Sparrows Corporation & its events, under the following terms & conditions: I acknowledge that I am 16 years old or older & medically & physically fit to volunteer. I voluntarily agree to work & acknowledge that MY service is provided strictly on a volunteer basis, without any pay or compensation of any kind, & without any liability of any nature on behalf of Steadfast Sparrows Corporation. I agree that my participation entails known & unanticipated risks that could result in physical or emotional injury, damage to me, to my property, or to third parties. These risks may include but are not limited to: Tripping, falling, scraping, bruising, scratched, bitten by insect or other, sunburn or other. I agree all services performed are at my own risk. I agree to assume the risk that may result from my participation. I certify that I have adequate insurance to cover any injury or damage I may suffer while participating, or else agree to bear the costs of such injury or damage myself. I authorize Steadfast Sparrows Corporation, its publishers, licensees & assignees, permission to use & reproduce still photographs & /or film footage taken of me in whole or in part, with or without names, for editorial, trade or advertising purposes. I also confirm that I waive the need to view photographs & /or film for "inspections of any kind" & all claims arising from such use for any additional compensation, damages, & invasion of privacy.

Medical Consent: The Participant, or the parent or guardian of Participant, signing below does hereby consent to all emergency mental, medical & surgical treatments, including anesthesia & operations, which may be deemed advisable by any treating medical professional, doctor, & /or surgeons. The intention hereby being to grant authority to administer & perform all examinations, treatments, anesthetics, operations & diagnostic procedures which may now or during the course of the patient’s care be deemed advisable & necessary. I also agree that if the Participant is admitted to a hospital, he or she is to remain there until the treating physician recommends discharge. 

In consideration of promises set forth in this Agreement. I agree that Steadfast Sparrows Corporation its agents, anyone involved, team, staff, other volunteers, children participating in camp or employees, shall not be liable or responsible for & shall be saved, held harmless, released, & indemnified by volunteer from & against any & all suits, actions, losses, damages, claims, or liability of any character, type, or description, including but not limited to all expenses of litigation, court costs, & attorney fees for injury or death to any person, or injury to any property received or sustained by any person or persons or property arising out of, or occasioned by, directly or indirectly, the participation of volunteer in activities arising out of this agreement, including claims & damages arising in whole or in part from the negligence of the Steadfast Sparrows Corporation, its agents or employees, etc.. It is the express intent of volunteer that the indemnity in this agreement is an indemnity extended by volunteer to indemnify & protect the Steadfast Sparrows Corporation from any & all claims or actions, as set forth above, of any kind, arising directly or indirectly from my volunteer services, regardless of whether such claims or actions are founded in whole or in part upon alleged negligence negligence of Steadfast Sparrows Corporation, its representatives, agents, or employees. In making this Agreement, volunteer relies wholly upon his/her own judgment, belief & knowledge & has not been influenced to any extent whatsoever by any representations or statements not contained in this Agreement. I have read this Volunteer Agreement, & fully understand all of its terms. I understand that I have given up substantial rights by signing it & have signed it freely & voluntarily without any inducement, assurance or guarantee being made to me & intend my signature to be a complete & unconditional release of all liability to the greatest extent allowed by law.
