DI​NO KID CAMP

Camp See Me Application (Prospective Kids):

Meds go in a large Ziploc with child’s name/DOB. Only original packaging with camper’s name and current dosage. No meds will be given unless in original packaging per TX DSHS. If your child requires inhalers/antidotes for allergies (prescribed by doctor) have them bring at least two (2) to camp. Meds must be registered with the Med. Professional. One (1) will be kept by camper and one (1) given to the Medical Professional. Special cases must be discussed with Med. Professional.

I, the undersigned parent or guardian, hereby consent to my child participating in Summer Camp at Dinosaur Valley State Park in Glen Rose, TX an event sponsored by Steadfast Sparrows Corporation's Camp See Me in November 2026. I certify that my child is able to participate in all activities including but not limited to:

• Playground

• Arts & Crafts

• Swimming (Safety gear will be provided)

• Hiking

• Fishing

• Group-Time

• Camping under the stars

• Real Life Dinosaur Tracks

• Real Ranger Kid Badges!

• Barbecue & Fire Rings for S'mores & Meals

• Wilderness Skills Certificate

• Fire Safety Certificate

• ​Water Safety Certificate

• Bible Study about Dinosaurs & how they once roamed the earth.​​

Liability Release, Medical & Indemnity Agreement:

As consideration for the acceptance of the above-references child as a Participant in the program/event, administered by Steadfast Sparrows Corporation's Camp See Me, the undersigned does hereby agree to the following: 

• I, individually as Participant, or I/we as parent(s) and/or natural/legal guardian(s) of my/our child named as the Participant herein, understand and recognize that there is always an inherent risk of bodily injury and harm associated with, but not limited to, camping activities, Playground, Arts & Crafts, Swimming (Safety gear will be provided), Hiking, Fishing, Group-Time, Camping under the stars, Real Life Dinosaur Tracks, Real Ranger Kid Badges! Barbecue & Fire Rings for S'mores & Meals, Wilderness Skills Certificate, Fire Safety Certificate, ​Water Safety Certificate, Bible Study about Dinosaurs & how they once roamed the earth., etc. and all other activities associated with the Program and activities, including those risks arising from accidents, other participants, and the forces of nature.   

• I, individually as Participant, or I/we as parent(s) and/or natural/legal guardian(s) of my/our child named as the Participant herein,  further understand that no warranties or representations of any kind have been made by any National parks or Steadfast Sparrows Corporation's Camp See Me, its employees, volunteers, agents, officers, directors, successors, or assigns, along with the sponsors of the Program, regarding any of these activities or the Program and activities.  

• I, individually as Participant, or I/we as parent(s) and/or natural/legal guardian(s) of my/our child named as the Participant herein, assume(s) the risk(s) of personal injury, accidents and/or illness, in consideration of, and in recognition of the inherent risks of the activity associated with Program, and/or participation in the Program and activities, on behalf of myself, and/or on behalf my/our child named as a Participant. 

Medical Consent: The Participant, or the parent or guardian of Participant, signing below does hereby consent to all emergency mental, medical and surgical treatments, including anesthesia and operations, which may be deemed advisable by any treating medical professional, doctor, and/or surgeons. The intention hereby being to grant authority to administer and perform all examinations, treatments, anesthetics, operations and diagnostic procedures which may now or during the course of the patient’s care be deemed advisable and necessary. I also agree that if the Participant is admitted to a hospital, he or she is to remain there until the treating physician recommends discharge. 

Photo/Video/Audio Consent: I, individually as Participant, or I/we as parent(s) and/or natural/legal guardian(s) of my/our child named as the Participant herein,  do hereby consent to the use by Steadfast Sparrows Corporation's Camp See Me, of Participant’s image, voice, or both, in (1) videos, photographs, audio tapes, or other forms of media created by Steadfast Sparrows Corporation's Camp See Me, or any other third party; and (2) and in videos, photographs, audio tapes, or other forms of media reproduced either in whole or in part from the videos, photographs,  audio tape, or other media described herein; regardless of whether these materials are used for fundraising, advertising, publicity, or any other purpose on behalf of Steadfast Sparrows Corporation's Camp See Me. In addition, I waive all claims to compensation or damages based on the use of my image or voice, or both by Steadfast Sparrows Corporation's Camp See Me. I also waive any right to inspect or approve any finished photographs or videos or audio tapes. I agree that all such portraits, pictures, photographs, videos, audio recordings, or other forms of media, and any reproductions thereof, and all plates, negatives, recording tapes, and digital files shall remain the property of Steadfast Sparrows Corporation's Camp See Me, unless otherwise notes. I understand that this consent is perpetual, that I may not revoke it, and that it is binding on my heirs and assigns.  

I, INDIVIDUALLY, OR I/WE AS PARENT(S) AND /OR NATURAL, LEGAL GUARDIAN(S) OF MY/OUR CHILD NAMED AS A PARTICIPANT, AGREE, ON BEHALF OF MYSELF, MY CHILD, MY/OUR HEIRS, REPRESENTATIVES, SUCCESSORS, EXECUTORS, ADMINISTRATORS AND ASSIGNS, DO HEREBY RELEASE, WAIVE, DISCHARGE, HOLD HARMLESS, ACQUIT, AND AGREE NOT TO SUE ANY NATIONAL PARKS, Steadfast Sparrows Corporation, ALL OF its EMPLOYEES, OFFICERS, DIRECTORS, STAFF, VOLUNTEERS, AGENTS, sponsors of the program, AND REPRESENTATIVES (COLLECTIVELY, THE “RELEASED PARTIES”), WITH RESPECT TO any and ALL liability, claim(s), demand(s), cause(s) of action, damage(s), loss or expense (including court costs and reasonable attorneys fees) of any kind or nature WHATSOEVER WHICH MY CHILD, named as PARTICIPANT, OR I MAY HAVE AGAINST THE RELEASED PARTIES ON ACCOUNT OF ANY PERSONAL INJURY, PROPERTY DAMAGE, DEATH OR ACCIDENT OF ANY KIND ARISING OUT OF OR IN ANY WAY CONNECTED WITH THE Program and activities, TRANSPORTING MY CHILD TO AND/OR FROM THE Program and activities, AND/OR PARTICIPATION IN ANY The Program and activities. 

I, INDIVIDUALLY, OR I/WE AS PARENT(S) AND /OR NATURAL, LEGAL GUARDIAN(S) OF MY/OUR CHILD AGREE, ON BEHALF OF MYSELF, MY CHILD, MY/OUR HEIRS, REPRESENTATIVES, SUCCESSORS, EXECUTORS, ADMINISTRATORS AND ASSIGNS, AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASED PARTIES FOR ANY AND ALL LIABILITIES, LOSSES, CLAIMS OF ANY KIND, PERSONAL INJURY, EXPENSES, PROPERTY LOSS, PROPERTY DAMAGE, ECONOMIC LOSS, PERSONAL OR BODILY INJURY, DEATH, OR ANY OTHER LIABILITY WHETHER OR NOT THE ACCIDENT, PROPERTY LOSS, PROPERTY DAMAGE, ECONOMIC LOSS, PERSONAL INJURY OR DEATH WAS THE FAULT OF OR WAS CAUSED, IN WHOLE OR IN PART, BY THE ALLEGED OR ACTUAL NEGLIGENCE OR CONTRIBUTORY NEGLIGENCE OF MY CHILD’S ACTIONS, MY OWN ACTIONS, THE ACTIONS OF THE RELEASED PARTIES, OR THE ALLEGED OR ACTUAL NEGLIGENCE, GROSS NEGLIGENCE, OR CONTRIBUTORY NEGLIGENCE OF ANY OFFICER, DIRECTOR, AGENT, EMPLOYEE, VOLUNTEER, SPONSORS OF THE PROGRAM, OR REPRESENTATIVE OF THE RELEASED PARTIES.   

I/we further understand that this release agreement binds my survivors, my family members, and/or spouse, my heirs, my assigns, and my personal representatives. This Agreement constitutes the entire agreement between Steadfast Sparrows Corporation's Camp See Me, and the undersigned regarding the subjects covered herein. There exist no further oral or written representations, promises, assurances or statements of any kind affecting this Agreement except those which are expressly set forth in this document or referenced herein. If Steadfast Sparrows Corporation's Camp See Me, is required to resort to legal action to enforce this Agreement, you will be responsible for all attorneys’ fees and costs related to such enforcement. This Agreement may be signed and delivered by electronic means, which shall have the same force and effect as an original signature. All terms are governed by Texas law and is intended to be as broad and inclusive as Texas law permits. Should any clause conflict with Texas law, only that clause will be null and void and the remainder of this document shall stay in full force and effect at all times, now or in the future.  

VOLUNTARY AND KNOWING CONSENT: 

I HAVE READ THIS DOCUMENT. I UNDERSTAND IT IS A WAIVER AND RELEASE OF ALL CLAIMS. I UNDERSTAND I ASSUME ALL RISK INHERENT IN PARTICIPATING IN the PROGRAM OR RELATED  EVENT(S) SPONSORED OR HOSTED BY Steadfast Sparrows Corporation's Camp See Me, HEREIN, INCLUDING the Program and activities. I VOLUNTARILY SIGNED MY NAME EVIDENCING MY ACCEPTANCE OF ALL OF THE AGREEMENTS CONTAINED IN THIS DOCUMENT.
